This means the pills don&#8217;t contain the full dose printed on the package, according to the FDA&#8217;s <a href=\"https:\/\/www.accessdata.fda.gov\/scripts\/ires\/index.cfm?Event=99399\" target=\"_blank\" rel=\"noreferrer noopener\">enforcement report<\/a>.<\/p>\n<p>An FDA Class II recall is considered a medium-risk recall that may lead to temporary or reversible health problems, with a low probability of serious harm. <\/p>\n<p>The recall includes seven strengths, from 25 to 150 micrograms (mcg). Lupin Pharmaceuticals, Inc. manufactured the tablets, according to the packaging labels included in a letter to customers that was shared with Healthline.<\/p>\n<p>Major Pharmaceuticals and Cardinal Health distributed them.<\/p>\n<p>Major Pharmaceuticals <a href=\"https:\/\/www.major-rugby.com\/major-news\" target=\"_blank\" rel=\"noreferrer noopener\">told<\/a> its wholesale customers in a letter dated July 10 that the recall began &#8220;due to out of specification results obtained during routine stability testing for Assay.&#8221; In this case, the <a href=\"https:\/\/www.cancer.gov\/publications\/dictionaries\/cancer-terms\/def\/assay\" target=\"_blank\" rel=\"noreferrer noopener\">assay<\/a> measured how much active drug a tablet actually holds.<\/p>\n<p>That letter, which was shared with Healthline, stated that the use of the product \u201cis not likely to cause adverse health effects.&#8221; It also told its wholesale customers their cooperation was \u201cnecessary to prevent patient harm.\u201d<\/p>\n<h2><strong>Which thyroid products are being recalled?<\/strong><\/h2>\n<p>The recall covers 12 products. The following lots were distributed by Major Pharmaceuticals:<\/p>\n<ul>\n<li>25 mcg: Lot N02212, NDC 0904-6949-61, Exp. 07\/2026&nbsp;<\/li>\n<li>50 mcg: Lot N02200, NDC 0904-6950-61, Exp. 10\/2026<\/li>\n<li>75 mcg: Lot N02172, NDC 0904-6951-61, Exp. 07\/2026<\/li>\n<li>75 mcg: Lot N02296, NDC 0904-6951-61, Exp. 12\/2026<\/li>\n<li>75 mcg: Lot N02288, NDC 0904-6951-61, Exp. 12\/2026<\/li>\n<li>88 mcg: Lot N02310, NDC 0904-6952-61, Exp. 01\/2027<\/li>\n<li>112 mcg: Lot N02203, NDC 0904-6954-61, Exp. 09\/2026<\/li>\n<li>125 mcg: Lot N02266, NDC 0904-6955-61, Exp. 11\/2026<\/li>\n<li>150 mcg: Lot N02167, NDC 0904-6956-61, Exp. 07\/2026<\/li>\n<\/ul>\n<p>Cardinal Health distributed five of the seven strengths under a second set of numbers and in different packaging.<\/p>\n<ul>\n<li>25 mcg: Bag lots N02212A and N02212B, Bag NDC 55154-3558-0; Blister lot N02212, Blister NDC 0904-6949-61, Exp. 07\/2026<\/li>\n<li>50 mcg: Bag lots N02200A and N02200B, Bag NDC 55154-3559-0; Blister lot N02200, Blister NDC 0904-6950-61, Exp. 10\/2026<\/li>\n<li>75 mcg: Bag lots N02172A and N02172B, Bag NDC 55154-3560-0; Blister lot N02172, Blister NDC 0904-6951-61, Exp. 07\/2026<\/li>\n<li>75 mcg: Bag lots N02288A and N02288B, Bag NDC 55154-3560-0; Blister lot N02288, Blister NDC 0904-6951-61, Exp. 12\/2026<\/li>\n<li>125 mcg: Bag lots N02266A and N02266B, Bag NDC 55154-3562-0; Blister lot N02266, Blister NDC 0904-6955-61, Exp. 11\/2026<\/li>\n<li>150 mcg: Bag lot N02167A, Bag NDC 55154-3563-0; Blister lot N02167, Blister NDC 0904-6956-61, Exp. 07\/2026<\/li>\n<\/ul>\n<p>The tablets started shipping on January 22, 2025, and some of the affected lots do not expire until January 2027, according to Major Pharmaceuticals.<\/p>\n<h2><strong>How to check if your thyroid prescription is recalled<\/strong><\/h2>\n<p>There&#8217;s no need to stop taking your medication. The FDA <a href=\"https:\/\/www.fda.gov\/drugs\/drug-recalls\/understanding-drug-recalls-what-know-and-what-do\" target=\"_blank\" rel=\"noreferrer noopener\">states<\/a> that stopping a recalled medication can sometimes cause more harm than continuing it. Talk with your doctor or pharmacist about any concerns.<\/p>\n<p>Because most pharmacies pour tablets into their own vials, the recall details won&#8217;t be on your bottle. Here&#8217;s how to check:<\/p>\n<ul>\n<li>Call the pharmacy where you last refilled your prescription.<\/li>\n<li>Ask for the manufacturer, NDC number, and lot number on file.<\/li>\n<li>Compare those against the recall tables below.<\/li>\n<\/ul>\n<p>&#8220;The overly cautious thing to do is simply check your <a href=\"https:\/\/www.healthline.com\/health\/optimal-thyroid-levels\">thyroid hormone levels<\/a>,\u201d said <a href=\"https:\/\/www.providence.org\/doctors\/family-medicine\/ca\/santa-monica\/david-cutler-1255368338\" target=\"_blank\" rel=\"noreferrer noopener\">David Cutler<\/a>, MD, a family medicine physician at Providence Saint John&#8217;s Health Center in Santa Monica, CA.<\/p>\n<p>\u201cIf you&#8217;re concerned that your thyroid hormone level might be low because you&#8217;re taking a subpotent product, simply check your thyroid hormone levels with a <a href=\"\/health\/t4-test\">blood test<\/a> to get reassurance that your thyroid hormone levels are okay,\u201d he told Healthline.<\/p>\n<p>Some state regulators have forwarded the recall to pharmacies they license.<\/p>\n<p>The California State Board of Pharmacy went further in its <a href=\"https:\/\/www.pharmacy.ca.gov\/about\/recall_alerts\/071526_major.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">July 15 notice<\/a>, urging pharmacies across the state to review their own quality assurance and recall procedures to address any gaps.<\/p>\n<h2><strong>Less potent doses may affect some people<\/strong><\/h2>\n<p>A spokesperson for the FDA told Healthline that a Class II recall indicates &#8220;use of or exposure to a violative product may cause temporary or medically reversible adverse health consequences,&#8221; or that &#8220;the probability of serious adverse health consequences is remote.&#8221;<\/p>\n<p>In plain terms, lasting harm is unlikely, according to the FDA. <\/p>\n<p>Both the <a href=\"https:\/\/www.fda.gov\/drugs\/drug-recalls\/understanding-drug-recalls-what-know-and-what-do\" target=\"_blank\" rel=\"noreferrer noopener\">FDA<\/a> and Major Pharmaceuticals said this recall is limited to the retail level. That means pharmacies and health systems that dispense prescriptions to patients are not required to contact them. <\/p>\n<p>Instead, the classification tells pharmacies and distributors to pull the products from their shelves.<\/p>\n<p>Still, a subpotent dose can affect some individuals. Cutler said those who take a weaker tablet than prescribed could develop <a href=\"\/health\/hypothyroidism\/symptoms-treatments-more\" target=\"_blank\" rel=\"noreferrer noopener\">low thyroid function<\/a>. Symptoms may include:<\/p>\n<ul>\n<li><a href=\"https:\/\/www.healthline.com\/health\/hypothyroidism-how-to-lose-weight\">weight gain<\/a><\/li>\n<li><a href=\"http:\/\/healthline.com\/health\/6-natural-remedies-constipation\/\">constipation<\/a><\/li>\n<li><a href=\"https:\/\/www.healthline.com\/health\/fatigue\">fatigue<\/a><\/li>\n<li>hair loss<\/li>\n<\/ul>\n<p>The risk of lasting damage is low, he said, because doctors monitor thyroid patients through regular blood work. &#8220;You&#8217;ll pick it up before there are any significant adverse effects of either a low or high level,\u201d Cutler said.<\/p>\n<h2><strong>What does levothyroxine treat<\/strong>?<\/h2>\n<p>Thyroid hormones help control <a href=\"\/health\/hypothyroidism\/managing-your-weight\">metabolism<\/a>, growth, and other bodily functions, according to a <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK500006\/\" target=\"_blank\" rel=\"noreferrer noopener\">review<\/a> published by the National Library of Medicine. <\/p>\n<p>Levothyroxine sodium is a synthetic version of <a href=\"\/health\/t4-test\" target=\"_blank\" rel=\"noreferrer noopener\">T4<\/a>, one of the two main hormones produced by the <a href=\"\/health\/thyroid-nodule\">thyroid gland<\/a>.<\/p>\n<p>&#8220;It is for people who have low thyroid functions to replace the thyroid hormone,&#8221; Cutler said.<\/p>\n<p>Levothyroxine sodium is approved for adults and children for two uses:<\/p>\n<ul>\n<li><strong><a href=\"\/health\/hypothyroidism\/symptoms-treatments-more\" target=\"_blank\" rel=\"noreferrer noopener\">Hypothyroidism<\/a> <\/strong>\u2014 where the tablets work as replacement therapy.<\/li>\n<li><strong><a href=\"\/health\/thyroid-cancer\" target=\"_blank\" rel=\"noreferrer noopener\">Thyroid cancer<\/a> <\/strong>\u2014&nbsp;where doctors use levothyroxine alongside surgery and <a href=\"\/health\/cancer\/iodine-treatment-for-thyroid-cancer\/\">radioactive iodine treatment<\/a> to <a href=\"https:\/\/dailymed.nlm.nih.gov\/dailymed\/drugInfo.cfm?setid=e95720f2-91c9-a6d0-f7d5-8bcb94d07bbc\" target=\"_blank\" rel=\"noreferrer noopener\">lower thyroid-stimulating hormone (TSH)<\/a>. <\/li>\n<\/ul>\n<p>Cutler said doctors check thyroid levels every three to six months to catch low or high function early.<\/p>\n<h2><strong>A pattern of potency recalls<\/strong><\/h2>\n<p>Levothyroxine has failed potency testing before. The <a href=\"https:\/\/www.fda.gov\/media\/77811\/download\" target=\"_blank\" rel=\"noreferrer noopener\">FDA documented<\/a> 10 recalls for subpotency, content uniformity, and stability between 1990 and 1997, before the drug required formal FDA approval.<\/p>\n<p>That changed in August 1997, when the FDA declared levothyroxine sodium tablets a &#8220;new drug&#8221; and required manufacturers to obtain formal approval to continue selling them, according to the federal health agency.<\/p>\n<p>The FDA moved to <a href=\"https:\/\/www.federalregister.gov\/documents\/2000\/04\/26\/00-10322\/prescription-drug-products-levothyroxine-sodium-extension-of-compliance-date\" target=\"_blank\" rel=\"noreferrer noopener\">standardize the drug<\/a> to reduce potency issues.<\/p>\n<p>Recalls have continued since. Accord Healthcare recalled 82,159 bottles of 25 mcg levothyroxine sodium tablets in June 2025, according to a <a href=\"https:\/\/www.accessdata.fda.gov\/scripts\/ires\/index.cfm?Product=214562\" target=\"_blank\" rel=\"noreferrer noopener\">separate FDA enforcement report<\/a>. The FDA also classified that recall as Class II for a subpotent drug.<\/p>\n<p>Cutler said the recall itself demonstrates the system working as intended because routine inspections caught the potency problem. <\/p>\n<p>With FDA-approved prescription drugs, &#8220;every step along the manufacturing pathway, whether it&#8217;s from the drug manufacturer, distribution or their being delivered at the pharmacy, they&#8217;re being monitored for their potency,&#8221; Cutler said.<\/p>\n<p class=\"inmi-source\">Source: <a
